Referrals accepted include:
Urgent management of venous thromboembolism (urgent referrals are ONLY accepted from the St Paul’s Hospital or Mount St. Joseph’s Emergency Department)
Duration and choice of anticoagulation for venous thromboembolism
Workup of thrombophilia or hypercoagulability
Primary or secondary thromboprophylaxis
Venous thromboembolism in unusual sites
Complicated anticoagulation management (e.g., cancer, APS, recurrent VTE, pregnancy)
History of heparin-induced thrombocytopenia (HIT)
Workup and management of arterial thromboembolism, excluding stroke/TIA and coronary artery disease
Perioperative anticoagulant management of patients undergoing surgery within the Providence Health Care (St. Paul’s Hospital or Mount St. Joseph’s Hospital)
Referrals NOT accepted include:
Diagnostic workup and management of stroke/TIA and coronary artery disease
Management of antiplatelet therapy, including perioperative recommendations
Perioperative anticoagulant management for patients undergoing surgery outside of the Providence Health Care, unless they are already attached to our clinic. These patients should be referred to their respective hospital or catchment area

Referral Process
All referrals should be faxed to 604-602-8652. Please follow the attached links to access referral forms and processes. Completion of the appropriate referral form is mandatory.
Urgent referral: Patients with a new diagnosis of acute deep vein thrombosis or pulmonary embolism in the emergency department at St. Paul’s Hospital will be seen at the Thrombosis Clinic within 72 hours. The referral is made through Cerner by the Emergency Physician.
