We are located the basement of the Columbia Tower at Royal Columbian Hospital in the Medical Daycare Unit.

 
Referrals accepted include:
  • Non-urgent management of venous thromboembolism (i.e. patients need to be already initiated on anticoagulation. If unsure about initiating anticoagulation or choice of anticoagulant for a patient, you can contact the on-call RCH Hematologist for phone advice)

  • Duration and choice of anticoagulation for venous thromboembolism

  • Workup of thrombophilia or hypercoagulability

  • Primary or secondary thromboprophylaxis

  • Venous thromboembolism in unusual sites

  • Complicated anticoagulation management (e.g., cancer, APS, recurrent VTE, pregnancy)

  • History of heparin-induced thrombocytopenia (HIT)

  • Perioperative anticoagulant management for patients with prior history of VTE who are having surgery at Royal Columbian Hospital or Eagle Ridge Hospital

Referrals NOT accepted include:
  • Urgent VTE management (i.e. initiation of anticoagulation for new VTE diagnosis)

  • Diagnostic workup and management of stroke/TIA, coronary artery disease and other arterial thrombosis

  • Management of antiplatelet therapy, including perioperative recommendations

  • Management of anticoagulation for cardiac indications, including perioperative management

  • Perioperative anticoagulant management for patients having surgery at sites other than Royal Columbian Hospital or Eagle Ridge Hospital, unless they are otherwise attached to our clinic. These patients should be referred to their respective hospital or catchment area
